Required Camper Forms
Annual Information Form Required for all programs.
Health History Form Every camper who stays overnight must have one on file with us.
Medical Authorization Form Another form every overnight camper must turn in.
Scheduling Form Everyone uses this except those from the Champaign-Urbana area, which has a different schedule.

  for those applying for financial aid so they can attend
  summer camp or Respite
